JOB SUMMARY:

Assists individuals served to lead a self-directed life and contribute to the community, assists with activities of daily living, encourages attitudes and behaviors that enhance community inclusion. This position is responsible for providing direct support, positive direction, and assistance to individuals in accordance with organizational policies and procedures; and personal outcome measures.

ESSENTIAL FUNCTIONS:
1. Provide direct support and positive direction in all daily living skills to individual served, including but not limited to, nutrition, shopping, laundry, housekeeping, simple household repairs, personal hygiene, health care, safety, communication, interpersonal relations, mobility, financial assistance, provide assistance in learning the skill of properly taking medication (s), and community integration.

2. Assist individuals served to understand and follow their physician’s orders regarding diets, repositioning, and range of motion, etc.

3. Employees providing overnight services will monitor and supervise individuals served according to each individual ICP/Personal Plan.

4. Facilitate communication and provide other necessary assistance to individuals served to attend medical appointments and therapies.

5. Maintain and submit all required documentation daily (e.g., time sheets, expense reports, progress notes, and other required daily paperwork).

6. Review and complete documentation for individual medication sheets, data collection sheets and communication logs as required.

7. Provide incidental transportation in privately owned vehicle, or by agency owned vehicle, for individuals served as needed. This may include transporting to and from Day Programming, Doctor Appointments, etc.  This may occur within City of Clinton, Clinton County or regionally such as Davenport or Iowa City. Conditions of driving may be during the day, night, and various weather conditions.

8. Must remain alert and awake while providing services throughout scheduled shift. No sleeping is allowed during any shift.

9. Adhere to work schedule unless prior arrangements are made through direct Supervisor or Designee.

10. Must be able to manage time effectively, to work alone and as a team member.

11. Communicate regularly with Supervisor, Coordinator and stakeholders as needed regarding individual’s progress, issues and concerns.

12. Other duties as assigned

QUALIFICATIONS:

1. Must be 18 years of age.
2. High School Diploma or equivalent required.

DUTIES AND RESPONSIBILITIES

  • Perform repetitive duties welding components on shafts to various sized chain linkages.  Work from shop orders, verbal instructions, and blueprints.  Use simple hand tools.
  • Set heat temperatures on welders.  Hand crank material into location.  Position attachment or flight and activate welder; inspect weld and make minor adjustments as necessary.
  • Inspect welds for proper location and making minor adjustments to position of welder gun.
  • Detect faulty operation of equipment or defective material and refer questionable cases to supervisor.  Maintain tools, equipment, and work area in proper manner.
  • Place finished chain on pallet.  When flight welding, must be able to lift chain from pallet to weld bench and back to pallet when completed.
  • Responsible for quality:  Accept training in blueprint reading, use of gauges and measuring tools common to manufacturing, along with related statistical process control principles and other quality control procedures.
  • Must be knowledgeable in and practice all safety practices/procedures.
  • Other duties as assigned.
