JOB SUMMARY: 

The Direct Support is responsible for providing assistance in daily living tasks, personal care and support, and ensuring the personal safety and welfare of physically disabled, medically and behaviorally challenged individuals in a residential setting while serving as an effective role model.

*ESSENTIAL DUTIES AND RESPONSIBILITIES:

  • Demonstrate knowledge, understanding, and implementation of the person’s individual service plan (IPP).
  • Models and use good decision-making skills and accesses appropriate resources to assist in developing quality services.
  • Gather and record client progress within the program.
  • Effectively implement person centered planning techniques.
  • Empathize with clients.
  • Maintain a high degree of confidentiality in working with clients and programs.
  • Model and display appropriate conduct during program hours.
  • Implement household tasks and social activities as scheduled.
  • Maintain open communication with SCL Manager to inform team members on client progress with goals, medical and/or social needs.
  • Transport clients to and from appointments/outings.
  • Attend mandatory SCL staff meetings & in-service training assigned by CROSSROADS, INC.
  • Be respectful and considerate with family members, caretakers, neighbors, case workers and other involved individuals.
  • Assist clients with financial management, including personal/grocery shopping, making deposits/withdrawals from personal accounts, etc.
  • Ability to use a checkbook and maintain a balance for clients if not self-sufficient.
  • Be responsible for completing written reports of medical appointments.
  • Complete electronic documentation timely and accurately.
  • Provide accurate and timely data and records of residents’ goals and behavior incidents.

ADDITIONAL DUTIES AND RESPONSIBILITIES:

  • Communicate effectively with the entire Supported Community Team.
  • Abide by HCBS guidelines as indicated in the HCBS manual available to you per request.
  • Follow all safety regulations, Dept. of Health and OSHA requirements and communicate necessary components for the clients to understand.
  • Assist with the assessment of client needs to be presented in regular team meetings.
  • Order prescription and non-prescription medications for clients, if needed.
  • Check in medications from pharmacy and administer medication, First Aid, and/or CPR if necessary.
  • Awareness and follow-up by filling out request forms for maintenance and safety of the site.

MINIMUM QUALIFICATIONS:

  • A valid driver’s license and auto insurance are required.
  • Background checks showing no evidence of founded abuse or criminal activity.
  • Receive and maintain certification in Medication Management, CPR, Mandatory Dependent Adult Abuse, Safety-care training, or any other training as required within the first 6 months of employment. Ability to complete all required Relias training within the first 30 days of employment.

Preferred Experience: One years’ experience working with individuals with mental and/or physical disabilities.

Physical Demands: This position requires physical abilities that include one or more of the following (this is not a comprehensive list): lifting 25 pounds occasionally, frequent bending, stooping, kneeling motions, physical interventions.

JOB TYPE:

  • Full-time
  • $15 - $19.60/Hr.
  • Average 40 hours (Possible overtime)
